三 | 《通知》要求,各地要按照国家服务规范要求,明确慢阻肺病患者健康服务内容、流程等,积极发挥中医药在其中的作用,并将其纳入家庭医生签约服务。加强紧密型县域医共体牵头医院对辖区内基层医疗卫生机构的技术支持和指导,明确转诊渠道,并做好后续追踪随访。 《通知》提出,要引导城乡居民加强自我体重管理,对慢性病患者和老年人提供分类分级健康服务。对体检中发现体重异常或有其他基础疾病的老年人,适当增加联系频次,重点了解健康状况,提供健康教育、健康咨询,指导转诊等;对病情不稳定及有严重并发症的,在服务规范要求的基础上视情增加随访频次,重点了解疾病控制情况,指导用药,提供有针对性膳食和运动干预建议,必要时指导转诊等。 2024年,我省基本公共卫生服务注重强化65岁及以上老年人健康服务,在现有老年人免费健康体检项目基础上,鼓励各地因地制宜结合基层医疗卫生机构服务能力、老年人健康需求等实际,通过优化家庭医生签约服务包、“健康积分”兑换等方式,丰富老年人健康体检项目,推进开展老年人认知功能初筛服务,关注辖区高龄失能老年群体。积极引导在基层机构之外开展体检的65岁及以上老年人在就近基层机构获得包括中医药在内的连续健康管理服务。 责任编辑:刘春阳。
